CTR Physician Survey
Ohio Revised Code 4731.30 and Ohio Administrative Code 4731-32-03 require each physician with a CTR to submit an annual report to the Medical Board. To collect the annual report data, the Medical Board emailed a survey to all certified physicians for the physicians to give their professional opinion on the effectiveness of medical marijuana for the conditions for which it has been recommended.
The board has extended the deadline to complete the CTR physician survey until August 31, 2022.
As a reminder, failure to adhere to this annual reporting requirement could lead to formal disciplinary action against you, including possible suspension or revocation of your medical license, and a fine of up to $20,000 for failure to comply with the law.
To remain in compliance with Ohio law, physicians with a CTR must respond to the 2022 survey even if they have not made any recommendations. Click here to take the survey.
If you have already taken the survey, no further action is required. Please do not submit the survey more than once.

Email Address Reminder
When registering patients and caregivers, please be sure to enter their email addresses correctly. Any error in the email address can cause significant delay in the registration process, as the activation link will not be sent to the correct inbox. Physicians now have the capability to select “Regenerate Registration Email” on the User Account tab. This feature may be useful if it is necessary to amend a patient’s or caregiver’s email address.
Patient Renewal
Patients seeking to renew their medical marijuana registration will not receive another “activation” email upon receipt of their new recommendation. Within 30 days of the registration expiration date, patients can visit www.ohiomedicalmarijuanaregistry.com and login with their registered email address and password to renew the registration. Please make sure all communication to patients includes the proper procedure for renewing their registration. Detailed instructions can be found here.
